SpaghettiKart is a port of the source code of Mario Kart 64, which was first released in 1996, allowing you to play this Nintendo 64 classic natively on PC. The software doesn't include any copyrighted material, so it is up to players to find and use a legal copy of the original US version to play the game correctly.
First get a compatible ROM
SpaghettiKart is very easy to set up, but you must follow a series of steps to the letter. The first step is to get a Mario Kart 64 US ROM. To do this, it's best to dump your own ROM cartridge, but you can, of course, download it from a website. In any case, the most important thing is to make sure that it's the US version and that its SHA-1 is the following: 579C48E211AE952530FFC8738709F078D5DD215E.
Generate the O2R file and start playing
The second step before you can start playing is to generate the .O2R file. To do this, simply run Spaghettify.exe and select the game ROM in .Z64 format. In just a few seconds, the mk64.O2R file will be generated, and the game will start. If the program crashes or quits unexpectedly when trying to generate the .O2R file, you can most likely fix it by simply moving the game folder from the Downloads directory. Many problems are solved by placing the SpaghettiKart folder directly in the C: root drive.

Once you start the game, you'll be able to access its Settings by simply pressing the ESC key. From here, you can adjust the game's resolution, the frames per second, the music and effects volume and the controls. You can even activate cheats and mods. After all, as it's a native PC port, you can enjoy all the advantages that an emulated version might not have.
